Registered Nurse MDS Coordinator

Axiom Gardens of Mount Vernon is seeking a Registered Nurse MDS Coordinator to join our long-term care team. In this role, you will coordinate the Minimum Data Set (MDS) process, develop individualized care plans, and support our interdisciplinary team in delivering comprehensive resident care.

Responsibilities

• Coordinate the MDS process, ensuring timely and accurate completion of assessments and care plans
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct assessments, gather data, and document resident information accurately and thoroughly
• Collaborate with healthcare professionals to identify and address resident needs
• Develop and maintain relationships with residents, families, and staff to promote open communication and collaboration
• Participate in quality improvement initiatives to enhance resident care and satisfaction
• Maintain accurate and up-to-date records and reports

Required Qualifications

• Current registered nurse (RN) license in the state of Illinois
• Strong clinical and assessment skills, with ability to accurately and thoroughly document resident information
• Excellent communication and interpersonal skills, with ability to work effectively with residents, families, and staff
• Strong organizational and time management skills, with ability to prioritize tasks and meet deadlines
• Ability to work in a fast-paced environment and adapt to changing priorities

Preferred Qualifications

• Experience in long-term care or post-acute care setting
