Registered Nurse Licensed (RN) in RIviera Beach 33404

The Lakes Home Care is seeking a Registered Nurse to join our Home Health team. In this role, you will assume responsibility and accountability for the application of the nursing process and the delivery of patient care. You will demonstrate the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Clinical Services.

Clinical Setting

You will work in a home health environment, caring for medically-fragile patients in your community. You can expect a 1 patient to 1 nurse ratio with tailored scheduling and an ideal drive time between patient visits.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
• Collect, analyze, and interpret data and information from health care members and document actual and/or potential nursing diagnoses
• Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
• Perform interventions according to identified priorities, plan of care, hospital policies, and patient care outcome standards
• Revise the plan of care according to evaluation, changes in medical plan of care, and effective or ineffective nursing interventions
• Use clinical judgment in evaluation activities to meet patient care needs, including establishing priorities
• Perform other Registered Nurse duties as assigned

Required Qualifications

• Current Registered Nurse (RN) license for Florida
• Current Health Certificate (per facility contract or state regulation)
• Current PPD or Chest X-Ray
• Current BLS card

Preferred Qualifications

• One year of prior Registered Nurse experience
