Elite Care Management is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) for home health care positions across multiple Illinois locations. In this role, you will provide one-on-one skilled nursing care to patients in their homes, working with a single patient throughout your shift. You will develop relationships with patients and their families, assess their individual needs, and deliver personalized care focused on enhancing quality of life and comfort.
Clinical Setting
This is home-based skilled nursing care. You will work independently in patients' homes, providing direct patient care and support with daily living activities.
Responsibilities
- Observe patients, record significant conditions and reactions, and notify supervisors or physicians of patient status and responses to medications, treatments, and significant incidents
- Administer medications and treatments as prescribed, and monitor and document the effectiveness of interventions
- Manage and direct patient care according to the established care plan, updating as needed
- Intervene in response to changes in patient condition and report changes to the appropriate physician or supervisor
- Demonstrate understanding of organizational and nursing policies and procedures
- Demonstrate strong communication skills, both spoken and written
Schedule and Locations
Positions are available in three locations with varying employment types:
Wilmington (Part-Time & PRN)
- 6pm - 6am Sunday
- PRN shifts available for various weekdays and weekends
Orland Park (Full-Time, Part-Time & PRN)
- 7am - 7pm various weekdays and weekends
- 7pm - 7am various weeknights and weekend nights
- PRN shifts available for both shifts
Palos Park (PRN)
- 7am - 7pm weekends required
- 7pm - 7am weekends required
Compensation
- $35 - $40 per hour
- Weekly pay with direct deposit
Required Qualifications
- Valid driver's license
- Social Security card
- Auto insurance card
- Current CPR card
- Active RN or LPN state license
- TB test results: either a 2-step TB test or chest X-ray within the last 12 months, OR QuantiFERON gold blood work within the last 5 years
