Job Description

Activate Care is seeking a Registered Nurse to conduct in-person health assessments for dually eligible members across Massachusetts. This is a field-based position focused on assessment and documentation work, not ongoing case management. You will travel to members' homes and community settings to complete timely, accurate assessments and document findings for handoff to the health plan's care management team.

Clinical Setting

This role is part of a community-focused care coordination program serving populations with health-related social needs. You will work independently in members' homes and community settings across Massachusetts, engaging directly with diverse populations while respecting cultural and linguistic needs.

Responsibilities

  • Conduct in-person health assessments in members' homes and community settings, including Uniform Core, Functional, and Comprehensive Assessments evaluating physical, cognitive, and functional status
  • Document assessment findings accurately in the health plan's care management platform (GuidingCare) following established workflows
  • Send prompt SOAP notes to assigned Care Managers within 48 hours of each assessment
  • Submit complete documentation consistently meeting health plan quality standards
  • Resolve any audit or review findings within two business days
  • Follow field-safety guidelines during independent home visits
  • Escalate clinical or safety concerns appropriately and adhere to state mandated reporter laws for abuse, neglect, or exploitation
  • Maintain compliance with HIPAA privacy rules, NCQA standards, and applicable regulatory requirements
  • Complete all ongoing mandatory training to maintain platform access

Required Qualifications

  • Active Massachusetts RN license (or higher, such as NP) in good standing
  • Associate Degree in Nursing from an accredited program
  • Valid driver's license, reliable personal vehicle, and active auto insurance
  • One or more years of home health clinical experience
  • Sound clinical judgment with strong organizational and decision-making skills
  • Ability to work independently in members' homes and manage a daily field schedule
  • Proficiency with electronic documentation systems via secure virtual desktop environments
  • Ability to document accurately while managing multiple priorities and maintaining attention to detail
  • Proficiency with standard office tools and communication software
  • Strong interpersonal skills with ability to build trust with members and families
  • Cultural sensitivity and awareness when engaging diverse populations

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN)
  • One or more years of assessment experience
  • RAC-CT or similar certification
  • Experience with dual-eligible, Medicaid, or Medicare Managed Care populations

Travel and Work Environment

This is a mobile, field-based role requiring 70% or more travel to members' homes and community settings across your assigned area within Massachusetts. You must maintain a reliable personal vehicle and continuous auto insurance. Employment is contingent upon a clear driving record check and maintaining personal transportation.

In-home visits involve exposure to varied residential environments, changing weather, and extended periods of sitting, standing, or driving. Primary work hours fall during set daytime slots based on member availability, with optional evening or weekend scheduling depending on shift selection. There are zero weekend requirements.

Health Requirements

Annual influenza vaccination is required during flu season (October 1 through March 31) as a condition of employment per health plan policy. New hires must submit proof within 30 days. Reasonable accommodation requests will be evaluated per applicable law.
