The opportunity

Job Description

This position is a Registered Nurse I role in Primary Family Medicine, responsible for providing comprehensive nursing care to an assigned group of patients across all age groups, including older adolescents, young adults, adults, geriatric, and pediatric patients. You will assess patients, develop and implement individualized care plans, and collaborate with an interdisciplinary team to ensure quality outcomes. You may supervise LVNs/LPNs, nursing assistants, unit secretaries, and other clinical staff, and by assignment, may function as a charge nurse.

Responsibilities

  • Develop comprehensive nursing care plans addressing all significant patient needs and problems identified through nursing assessment
  • Prepare nursing elements of the interdisciplinary care plan prior to initial team conferences
  • Review and update nursing care plans collaboratively as changes occur
  • Implement patient care according to established nursing and interdisciplinary care plans
  • Set priorities based on patient acuity and needs
  • Provide safe and therapeutic care, seeking resources when appropriate
  • Safely perform and accurately document procedures consistent with established nursing policies and procedures
  • Evaluate patient responses to interventions and goal attainment in collaboration with the interdisciplinary team
  • Evaluate progress toward nursing goals and revise goals as needed on a weekly basis
  • Complete nursing progress notes for assigned patients daily
  • Complete nursing flow sheets for assigned patients daily
  • Assess education needs of patients and significant others and document their response to teaching
  • Utilize, adapt, and revise teaching resources to meet patient education needs
  • Explain procedures, tests, and disease processes to patients and families as needed
  • Act as a role model for healthcare students and team members
  • Serve as a preceptor to assigned orientees
  • Assist in the orientation of new staff by sharing knowledge and experience
  • Report and document accurately and concisely on patient conditions to appropriate persons
  • Maintain patient confidentiality
  • Apply verbal and nonverbal communication skills to maintain open, effective communication among care team members, patients, and significant others
  • Encourage direct discussion between staff members when differences occur and seek assistance to arbitrate differences as needed
  • Give and accept constructive feedback appropriately
  • Identify actual and potentially unsafe patient care practices, report them in the designated manner, and suggest alternatives
  • Meet expectations of applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders

Required Qualifications

  • Active RN license in state of employment or compact
  • BLS certification
  • Six months to one year of experience as a licensed RN or holder of an active Graduate Nurse permit
  • Graduate Nurse Permit (Associates with an unsuccessful licensure application or on the 75th day following the effective date of the temporary permit will be transferred to another position within CHRISTUS Health)

  • Bachelor of Science Degree in Nursing
