Join Hospice of Cincinnati as a Registered Nurse in our Float Pool, providing compassionate end-of-life care to patients and families during vulnerable moments. You will work independently in home-based and field settings across a seven-county region, serving as a trusted resource for patients, families, physicians, and the care team while maintaining comfort, dignity, and continuity of care.
Clinical Setting
This position serves patients receiving hospice care in Hamilton, Preble, Butler, Brown, Clermont, Clinton, and Warren counties. You will provide direct and indirect holistic care to patients and their families, collaborating with physicians and interdisciplinary healthcare team members.
Responsibilities
- Perform initial and ongoing assessments of patients and families, completing assessment tools and documenting findings per TriHealth guidelines
- Communicate assessment findings to other healthcare providers and include health counseling and teaching needs in assessments
- Plan individualized care for patients and families based on assessment, standards of care, and optimal outcomes
- Initiate and individualize appropriate patient care guidelines, clinical pathways, and plans of care
- Develop short and long-term goals with patients, families, and the healthcare team, including discharge planning
- Provide a safe, therapeutic environment that maintains patient autonomy, dignity, and rights while being sensitive to patient diversity
- Recognize emergency situations and take appropriate action
- Complete patient assignments and documentation within scheduled timeframes
- Base interventions on clinical data and desired outcomes, documenting accordingly
- Train and educate other staff and serve as an expert resource in specialty areas
- Carry out physician orders
- Evaluate plans of care based on optimal patient outcomes and document patient and family responses to care and teaching
- Collaborate with patients, families, and healthcare team members to revise plans of care as needed
- Supervise care delegated to other healthcare team members
- Perform technical skills according to policy, procedure, and accepted standards
- Safely administer medications and treatments, monitoring their effects
- Use all equipment in a safe and appropriate manner
- Identify areas for professional and self-improvement through self-assessment and peer review
- Share knowledge and skills with colleagues and provide constructive feedback
- Maintain awareness of continuous improvement changes and support them through active participation
Schedule
- Full-time position: 64 hours bi-weekly
- 7:00 AM to 7:30 PM
- Day shift with weekend and holiday rotation
- Travel required to multiple counties
Required Qualifications
- Active Registered Nurse (RN) license
- Associate's Degree or Diploma in Nursing
- Up to one year of clinical nursing experience
- Basic Life Support (BLS) certification
- Knowledge, judgment, and skills derived from principles of biological, physical, behavioral, social, and nursing sciences to meet complex healthcare needs at various stages of the life cycle
Education Requirements
- New hires are required to obtain a Bachelor of Science in Nursing (BSN) within five years of hire

Travel Requirements
This position requires travel to multiple counties: Hamilton, Preble, Butler, Brown, Clermont, Clinton, and Warren. Mileage reimbursement is provided at 0.76 per mile.
Working Conditions
This role involves frequent bending, consistent lifting of less than 10 pounds and up to 50+ pounds, consistent pushing and pulling, consistent reaching, consistent standing and walking, occasional sitting and kneeling, and rare climbing.
