Registered Nurse – Hospice Home Health -Lawrence MA

This position offers skilled hospice nursing care in patients' homes throughout the Lawrence, MA area. You will provide compassionate end-of-life care, assess patient conditions, implement physician-ordered interventions, and support patients and families during this critical time.

Clinical Setting

• Home-based hospice care
• Patient visits in private residences
• Collaboration with interdisciplinary hospice care teams

Responsibilities

• Provide skilled hospice nursing care according to physician orders and individualized care plans
• Assess patient conditions and implement appropriate nursing interventions
• Complete hospice visits, including Start of Care and follow-up assessments
• Monitor patient status and communicate changes to the care team and physicians
• Provide education and emotional support to patients and their families
• Maintain accurate and timely clinical documentation
• Collaborate with physicians and interdisciplinary team members

Schedule

• Monday through Friday, 8:00 AM to 5:00 PM
• 40 hours per week
• 13-week contract

Required Qualifications

• Minimum 1 year of recent hospice nursing experience
• Active, unencumbered Massachusetts RN license
• Current driver's license
• Reliable personal transportation for home visits
• Ability to travel between patient homes
• AHA BLS certification

Compensation and Benefits

• Up to $2,200 gross weekly
• Benefits available from day 1
