Registered Nurse - Home Health(Manhattan) - Mandarin OR Cantonese

Girling Home Care of NY is hiring Registered Nurses to provide home health services in Manhattan. This position requires fluency in Cantonese or Mandarin to effectively communicate with the patient population served. You will conduct comprehensive home visits, develop individualized care plans, and coordinate patient care within the community setting.

Clinical Setting

You will work in patients' homes throughout Manhattan, providing direct nursing care and health education. This role emphasizes community health and leverages community resources to support patients and families.

Responsibilities

• Complete initial patient and family assessments to determine home care needs, including comprehensive physical assessments and medical history
• Regularly re-evaluate patient nursing needs and adjust care accordingly
• Initiate and revise plans of care as patient status and needs change
• Use health assessment data to determine nursing diagnoses
• Develop care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions, including patient and family input
• Initiate appropriate preventive and rehabilitative nursing procedures
• Administer medications and treatments as prescribed by the physician
• Counsel patients and families in meeting nursing and related needs
• Provide health care instructions appropriate to assessment and plan of care
• Identify and implement discharge planning needs prior to patient discharge
• Serve as Case Manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseload
• Prepare clinical notes and update the primary physician when necessary and at least every sixty days

Required Qualifications

• Cantonese or Mandarin language proficiency
• Graduate of an accredited school of nursing with ADN or BSN degree
• Current New York State RN license and registration
• Personal vehicle for travel to patient homes

Preferred Qualifications

• Home care nursing experience
• Bilingual proficiency (in addition to Cantonese or Mandarin)

Employment Type and Compensation

Full Time or Fee For Service positions available.

Full Time Salary Range: $110,000 - $125,000

Fee for Service: $80 - $85 per visit. Visit rates may vary depending on location, type of visit, and population served.
