The opportunity

Job Description

Brooks Home Health is seeking a Registered Nurse to provide nursing care planning and management for patients in the Jacksonville area and surrounding communities. This is a full-time, weekends-only position in home health care, following the Florida Nurse Practice Act.

You will work collaboratively with a multidisciplinary team to deliver comprehensive, patient-centered care. This is a non-supervisory role that requires independent clinical judgment, strong communication skills, and the ability to manage complex patient situations.

Responsibilities

  • Conduct bio-psycho-social-spiritual assessments and identify actual and potential nursing problems
  • Develop individualized nursing plans of care within 24 hours, from admission through discharge planning
  • Establish patient-centered goals in collaboration with clients, families, and the rehabilitation team
  • Recognize and document subtle changes in patient condition and take appropriate nursing action
  • Function independently and effectively in emergency situations
  • Complete medication reconciliation at each visit, documenting changes, noncompliance, and side effects
  • Administer medications and treatments as ordered, evaluate patient response, and anticipate adverse reactions
  • Complete all OASIS documentation according to guidelines in a timely and effective manner
  • Implement nursing interventions based on scientific nursing theory and evidence-based practice guidelines
  • Modify the plan of care in response to changes in patient needs
  • Demonstrate therapeutic and professional communication with patients, families, and healthcare team members
  • Act as a patient advocate and participate in care conferences and interdisciplinary team meetings
  • Support orientation and preceptorship for new staff members
  • Maintain a clean and safe environment and report hazards
  • Participate in on-call rotations, weekend rotations, staff meetings, committees, and mandatory in-services
  • Adhere to universal precautions, patient rights policies, and professional standards of rehabilitation nursing practice

Schedule

  • Full-time, weekends only
  • Participation in on-call and weekend rotations based on office needs

Required Qualifications

  • Active Registered Nurse license from the State of Florida
  • Associate, Bachelor's, or Master's degree from an accredited school of nursing
  • Basic Life Support (BLS) certification
  • Responsibility for maintaining BLS certification and required continuing education
