Job Description

Vertus HealthCare is seeking experienced Registered Nurses to provide skilled nursing care to patients in their homes throughout Palm Beach County. This position offers flexible per diem opportunities for compassionate and dependable RNs.

Responsibilities

  • Provide skilled nursing visits based on the patient's plan of care
  • Complete OASIS, SOC, ROC, Recertification, and Discharge visits as assigned
  • Assess patient needs and coordinate care with the clinical team
  • Complete timely and accurate clinical documentation
  • Communicate with physicians, patients, families, and the healthcare team

Required Qualifications

  • Current Florida RN license
  • Reliable transportation
  • Strong clinical assessment and documentation skills

Preferred Qualifications

  • Home Health experience
  • OASIS experience
