Registered Nurse - Home Health *Up to $10,000 Sign on Bonus*

Silver Cross Hospital is seeking a Registered Nurse for Home Health Skilled Nursing to provide direct patient care in the home setting. This position offers up to a $10,000 sign-on bonus for experienced home health nurses.

Clinical Setting

You will deliver skilled nursing care to patients in their homes, working independently while maintaining strong clinical judgment and comprehensive patient assessment.

Responsibilities

• Provide direct patient care in the home setting
• Perform comprehensive patient assessments and documentation
• Administer infusions and manage wound care
• Perform phlebotomy procedures
• Maintain accurate OASIS C documentation
• Ensure compliance with state and federal regulations
• Coordinate care and communicate with the healthcare team
• Provide exceptional customer service to patients and families

Required Qualifications

• Current Illinois RN license
• CPR certification
• Previous home health nursing experience
• Strong clinical background with prior hospital experience
• Proficiency in phlebotomy, infusion administration, and wound care
• Knowledge of OASIS C documentation and state and federal regulations
• Basic computer skills
• Ability to work independently
• Valid Illinois driver's license
• Proof of automobile insurance
• Personally owned vehicle for home visits

Schedule

• Variable weekday and weekend shifts
• Weekends and holidays required
• On-call duties required

Compensation and Bonuses

• Pay range: $35.00 - $42.50 per hour
• $10,000 sign-on bonus for RNs with 2 or more years of home health RN experience
• $5,000 sign-on bonus for RNs with 2 or more years of RN experience who are new to home health
• Final pay rate may exceed the posted range based on licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, and market data
