Registered Nurse Home Health RN

Elara Caring is seeking a Registered Nurse Home Health to provide direct patient care in home settings. In this role, you will assess patients, develop and implement care plans, monitor patient conditions, and educate patients and families on their care and safety. You will work collaboratively with the care team to ensure quality, safe, and continuous patient care while maintaining HIPAA compliance.

Responsibilities

• Assess patients to determine their care needs
• Complete initial and ongoing patient assessments
• Provide nursing care according to the plan of care and physician orders
• Monitor patient conditions and report changes to the care team
• Educate patients and families on their care and safety
• Take appropriate action when patient conditions change
• Maintain patient and staff privacy in accordance with HIPAA
• Help ensure quality, safe, and continuous patient care

Required Qualifications

• Current State license as a Registered Nurse
• Graduate of an accredited Certificate, Diploma, Associates, or Baccalaureate School of Nursing
• One or more years of experience as a nurse in a clinical care setting
• Valid driver's license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
