Registered Nurse - Home Health

Puget Sound Home Health & Hospice is seeking Registered Nurses to join our Home Health team serving Pierce County, including Lakewood, University Place, Bonney Lake, Sumner, Puyallup, and Buckley. This full-time position offers the opportunity to build meaningful relationships with patients while providing skilled nursing care in their homes, combining independence with the support of a collaborative clinical team.

Responsibilities

• Assess patients in their homes and identify individual care needs
• Develop and manage personalized plans of care in collaboration with providers
• Provide skilled nursing care, education, and support to patients and families
• Coordinate care with therapists, physicians, and other members of the interdisciplinary team
• Monitor patient progress and adjust care as needs change
• Complete accurate, timely clinical documentation
• Participate in quality initiatives to improve patient care

Required Qualifications

• Current Registered Nurse (RN) license
• Current AHA BLS certification
• Valid driver's license, insured vehicle, and reliable transportation
• Ability to work independently while collaborating with a clinical team

Preferred Qualifications

• Home Health and/or Hospice experience

Compensation

• $45–$60 per hour, based on experience
• Comparable pay-per-visit rates available

Travel Requirements

You will use your personal vehicle to travel between patient homes. Mileage reimbursement is provided.
