Job Description

Personal Touch Home Care is seeking a Registered Nurse to provide skilled home health nursing services to adult, geriatric, and pediatric patients in Manhattan, NY. You will plan, organize, and direct comprehensive home care services, including patient assessments, care planning, medication administration, and coordination with physicians and interdisciplinary teams.

Clinical Setting

This is a fee-for-service position providing home-based nursing care. You will work independently in patients' homes, delivering skilled nursing services across multiple patient populations.

Responsibilities

  • Complete comprehensive assessments of patients and families to determine home care needs, including complete physical assessments and medical histories
  • Regularly re-evaluate patient nursing needs and revise plans of care as patient status changes
  • Develop individualized plans of care in collaboration with patients' physicians, establishing goals based on nursing diagnoses and incorporating therapeutic, preventive, and rehabilitative nursing actions
  • Include patients and families in the care planning process
  • Administer medications and treatments as prescribed by physicians
  • Provide health care instructions and counseling to patients and families
  • Perform high-tech nursing skills including indwelling and straight catheter insertion/removal, nephrostomy care, ostomy care, plural and abdominal catheter drainage care, infusion services, g-tubes, and negative pressure wound therapy
  • Assess patient needs and physical status at each skilled visit through health data assessment and patient interviews
  • Inform physicians of changes in patient condition, response to care, and progress; obtain and clarify orders for plan revisions
  • Prepare clinical notes and update primary physicians as necessary and at least every 60 days
  • Communicate with other healthcare providers to coordinate care
  • Identify and implement discharge planning needs
  • Instruct, supervise, and evaluate home health aide and personal care aide services every 14 days or as required by law
  • Instruct, supervise, and evaluate LPN/LVN care at least every 30 days
  • Participate in on-call duties as defined by company policy
  • Ensure arrangements for necessary equipment, supplies, and services
  • Coordinate care between home, community providers, and pediatric tertiary specialists

Required Qualifications

  • Bachelor's degree in Nursing from an accredited college or university
  • Current active and unrestricted Registered Nurse (RN) license in New York State
  • Valid driver's license
  • Current certification in Basic Life Support for Health Care Professionals
  • Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting
  • Excellent observation, verbal, and written communication skills
  • Problem-solving skills and basic math skills
  • Nursing skills per competency checklist
  • Basic knowledge of computer operating systems and software applications for communication and documentation
  • Working knowledge of all infection control procedures and safety precautions
  • Knowledge of the nurse's scope of practice and ability to apply professional theory, practice, and procedure
  • Ability to observe ethical and legal guidelines for safeguarding patient confidentiality

  • Pediatric nursing experience

Compensation

  • Standard visits: $50 to $125 per visit
  • OASIS visits and Start of Care (SOC): $100 per visit, plus $25 bonus if notes are completed within 24 hours
  • Follow-up and evaluation visits: $50 per visit, plus $25 bonus if notes are completed within 24 hours
