Signature Healthcare at Home is seeking a Registered Nurse to provide skilled nursing services in Skagit County, including Mount Vernon, Burlington, Sedro-Woolley, Bow, Anacortes, Hamilton, and Concrete. This home health position offers high autonomy and the opportunity to meet patients in their own environments with self-directed scheduling based on patient needs and territory.
Responsibilities
- Complete initial and ongoing comprehensive assessments of patients and their family/caregiver living situations to determine care needs, including full physical assessment and medical history
- Establish individualized plans of care in collaboration with the certifying medical provider based on comprehensive assessment and patient goals
- Regularly re-evaluate patient and family/caregiver needs and revise the plan of care as necessary when patient status and needs change
- Develop care plans that establish interventions and goals, including patients and family/caregivers in the planning process
- Work autonomously to initiate appropriate preventive and rehabilitative nursing procedures
- Administer medications and treatments as prescribed by the medical provider
- Provide health care instructions to patients as appropriate per assessment and plan
- Facilitate patients' efforts toward self-sufficiency or obtaining appropriate caregiving
- Act as Case Manager when assigned and coordinate patient care for assigned caseload
- Document patient services and status timely and accurately within professional practice standards and reimbursement requirements
- Communicate with medical providers regarding patient needs and report changes in patient condition; obtain and receive medical provider orders as required
- Teach patients and family/caregivers self-care techniques, medication, diet, and other instructions as ordered by the medical provider
- Provide health counseling with patients and families/caregivers and work in concert with the interdisciplinary team
- Attend required staff meetings, interdisciplinary team meetings, and care coordination activities
- Complete mandatory education requirements to maintain competence
- Coordinate patient care with LPN/LVN as assigned by Clinical Manager
Schedule
- Full-time position (32+ hours per week)
- Monday through Friday day shift
- Self-directed scheduling dependent on patient needs and territory
Travel Requirements
- Must have the ability to drive and/or have access to transportation for agency-related travel to community partners and patients
- Must have and maintain a valid driver's license
- Must maintain automobile insurance coverage
- Must have access to an automobile
- Mileage reimbursement provided
Required Qualifications
- Graduate of an accredited school of nursing
- Currently licensed as a Registered Nurse in Washington State
- Must be licensed and able to practice in Washington State
- Possesses and maintains CPR certification
- One to two years of recent acute care experience in an institutional setting
- One to two years of recent experience in home health
Preferred Qualifications
- Experience in home health or post-acute care
- Familiarity with Homecare Homebase EMR
Compensation and Benefits
- Hourly rate: $50-$55 (dependent on experience)
