Pathways Healthcare, a physician- and nurse practitioner-led home health and hospice organization, is seeking a Registered Nurse for Home Health to provide comprehensive, patient-centered care in patients' homes. This full-time position offers the opportunity to make a direct impact on patients' lives while working in a supportive team environment with consistent day-shift hours.
Clinical Setting
You will provide home health services across a service area including Cranston, Warwick, West Warwick, Coventry, East Greenwich, and Providence, Rhode Island, working from the main office location in Cranston. The organization's innovative Medical Rehabilitation Program offers patients a meaningful alternative to institutional rehabilitation, supporting healing at home with dignity and purpose.
Responsibilities
- Assess and evaluate patient status, writing and initiating plans of care
- Regularly re-evaluate patient and family/caregiver needs and revise the plan of care as necessary
- Develop care plans that establish goals based on nursing diagnosis and incorporate palliative nursing actions, including patients and families in the planning process
- Initiate appropriate preventive and rehabilitative nursing procedures
- Administer medications and treatments as prescribed by the physician
- Complete, maintain, and submit accurate and relevant clinical notes regarding patient condition and care provided
- Record pain and symptom management changes and outcomes as appropriate
- Communicate with physicians regarding patient needs and report changes in patient condition; obtain and receive physician orders as required
- Provide and maintain a safe environment for patients
- Attend interdisciplinary group meetings
- Participate in on-call duties
- Ensure arrangements for equipment and other necessary items and services are available
- Supervise ancillary personnel and delegate responsibilities when required
- Maintain and upgrade professional knowledge and practice skills through continuing education and in-service participation
- Fulfill requested and accepted case assignments
- Actively participate in quality assessment and performance improvement teams and activities
Required Qualifications
- Degree in Nursing from an accredited institution
- Active RN license
- Current BLS CPR certification
- Valid driver's license and reliable, insured vehicle
- Proficiency with electronic medical records
- Excellent bedside manner and communication skills
- Working knowledge of health care laws and regulations
Preferred Qualifications
- Two or more years of nursing experience, preferably in geriatrics

Compensation and Benefits
- Salary Range: $72,000 to $90,000 annually
