Registered Nurse (Home Health)

Pathways Healthcare, a physician- and nurse practitioner-led home health and hospice organization, is seeking a Registered Nurse to provide comprehensive, patient-centered in-home care. This full-time position offers a meaningful alternative to institutional rehabilitation, helping patients heal at home with dignity and purpose.

Clinical Setting

You will work in patients' homes throughout the Reston, Centreville, and surrounding areas, with the main office located in Springfield, Virginia. Pathways Healthcare specializes in innovative medical rehabilitation programs and hospice services.

Responsibilities

• Assess and evaluate patient status, writing and initiating plans of care
• Regularly re-evaluate patient and family/caregiver needs and revise the plan of care as necessary
• Develop care plans that establish goals based on nursing diagnosis and incorporate palliative nursing actions, including patients and families in the planning process
• Initiate appropriate preventive and rehabilitative nursing procedures
• Administer medications and treatments as prescribed by the physician
• Complete, maintain, and submit accurate clinical notes regarding patient condition and care provided
• Record pain and symptom management changes and outcomes
• Communicate with physicians regarding patient needs and report changes in patient condition; obtain and receive physician orders as required
• Provide and maintain a safe environment for patients
• Attend interdisciplinary group meetings
• Participate in on-call duties
• Ensure arrangements for equipment and necessary items and services are available
• Supervise ancillary personnel and delegate responsibilities when required
• Maintain and upgrade professional knowledge through continuing education and in-service participation
• Fulfill requested and accepted case assignments
• Actively participate in quality assessment and performance improvement teams and activities

Schedule

• Full-time position
• Monday through Friday with weekend rotation (not on-call)
• Consistent day-shift hours

Required Qualifications

• Active nursing license in Virginia (RN or LPN)
• Degree in Nursing from an accredited institution
• Proficiency with electronic medical records
• Excellent bedside manner and communication skills
• Working knowledge of health care laws and regulations
• Current BLS CPR certification
• Valid driver's license and reliable, insured vehicle

Preferred Qualifications

• Two or more years of nursing experience, preferably in geriatrics
