Registered Nurse Home Health Part Time (2934)

Brooks Home Health is seeking a Registered Nurse for part-time home health nursing in the Daytona and surrounding areas. In this non-supervisory position, you will provide comprehensive nursing care planning and management for patients in collaboration with a multidisciplinary team, following the Florida Nurse Practice Act.

Clinical Setting

You will work in home health settings throughout the Daytona area, providing direct patient care and coordinating with rehabilitation and clinical teams.

Responsibilities

• Conduct bio-psycho-social-spiritual assessments and identify actual and potential nursing or clinical problems
• Develop and implement comprehensive individualized nursing plans of care within 24 hours, from admission through discharge planning
• Demonstrate critical thinking in application of the nursing process
• Develop patient-centered goals in collaboration with clients, families, and the rehabilitation team that promote wellness, independence, and socialization
• Recognize and document subtle changes in patient condition, including complex patient care situations, and take appropriate nursing action
• Function independently and effectively in emergency situations and serve as a resource to other healthcare team members
• Complete all documentation according to guidelines and regulations
• Evaluate outcomes of care and revise the plan of care to achieve desired outcomes
• Plan and prioritize your own work and assigned staff work; delegate tasks appropriately; coordinate patient care efforts to be completed during designated shifts in a timely manner
• Complete medication reconciliation at each skilled nursing visit and update the clinical team and physician of any changes, noncompliance, or side effects
• Administer medications and treatments as ordered, evaluate patient response, document appropriately, and anticipate adverse reactions
• Develop and document teaching and discharge plans addressing complex patient and family needs; document progress according to outcomes
• Design and implement nursing plans of care and treatment strategies based on scientific nursing theory related to self-care and promotion of physical, psychosocial, and spiritual health
• Apply nursing research to clinical practice and utilize evidence-based practice guidelines and patient/family-centered interventions
• Independently complete all appropriate nursing skills and interventions according to specialty competency-based skills checklists; update and teach new skills
• Consistently deliver patient care according to the established plan of care; modify the plan in response to actual and anticipated changes in patient needs
• Complete all OASIS assessments according to guidelines in a timely and effective manner; maintain proficiency and competency through all updates
• Demonstrate therapeutic communication skills when interacting with patients and families
• Function as a patient advocate with other members of the healthcare team
• Maintain professional communication skills with all colleagues; facilitate a harmonious working environment
• Initiate formal and informal patient care conferences as needed to coordinate direct patient care efforts and optimize outcomes
• Collaborate with the interdisciplinary team in care planning and care delivery
• Participate in interdisciplinary team processes at team conferences, case conferences, and other team meetings
• Identify and intervene for peer, patient, and family education needs
• Support orientation for new staff members and ancillary personnel by acting as a preceptor
• Maintain professional development and skills appropriate to your practice and experience
• Promote professionalism based on the established scope and standards of Rehabilitation Nursing Practice and the American Nurses Association
• Participate in on-call and weekend rotations based on office needs
• Participate in all staff and team-based meetings and committees
• Meet all required mandatory in-services
• Adhere to policies and procedures specific to universal precautions, patient rights, attendance, conduct, grooming, and dress code
• Promote a positive image of Brooks Home Health to outside agencies and the public
• Participate in performance improvement activities as needed

Required Qualifications

• Active Registered Nurse license from the State of Florida
• Associate, Bachelor's, or Master's degree from an accredited school of nursing
• Basic Life Support (BLS) certification
• Responsibility for maintaining BLS certification and required continuing education and unit-specific credentials

• Admission nurse background or experience
