Alabama HomeCare of Cullman, part of LHC Group, is hiring a full-time Registered Nurse for Home Health. In this role, you will provide and direct nursing care to patients in their homes according to physician orders and applicable regulations. You will develop and implement individualized plans of care, supervise home health aides and licensed practical/vocational nurses, coordinate with the healthcare team, and maintain continuity of care through effective communication with physicians, patients, families, and community resources.
Responsibilities
- Initiate, develop, implement, and revise plans of care in collaboration with physicians and other healthcare professionals
- Supervise care provided by home health aides and licensed practical/vocational nurses, provide instruction, and assign tasks according to state and federal regulations
- Provide required supervisory visits
- Deliver high-quality clinical services within the scope of practice and infection control standards
- Complete comprehensive assessments including OASIS documentation and medication reconciliation accurately and timely
- Document patient visits per policy and payer requirements
- Make initial and comprehensive nursing evaluation visits to ensure patients meet home health eligibility and medical necessity guidelines
- Determine primary focus of care and develop plans of care within state guidelines with the physician
- Communicate relevant information timely and effectively with agency staff regarding patient care issues, visit assignments, schedule changes, orders, and coordination with other clinicians
- Communicate timely and effectively with physicians, patients, and family members to ensure quality care and service excellence
- Take direction from Clinical Director and Executive Director and complete assigned tasks timely
- Assist in orientation of new agency personnel and serve as a preceptor to other staff and students
- Participate in survey readiness activities and performance improvement plans
- Work to reduce unnecessary patient hospitalizations and improve patient safety
- Participate in on-call and weekend rotation as needed to meet patient needs
- Adhere to and participate in the agency's utilization management model
- Function professionally in any home situation regardless of patient age, race, creed, color, sex, disability, or financial condition
Required Qualifications
- Current and unrestricted RN licensure in Alabama
- Current driver's license
- Vehicle insurance
- Access to a dependable vehicle or public transportation
Preferred Qualifications
- Current CPR certification or ability to complete within 90 days of hire
- One or more years of home health experience
- Ability to work independently
- Solid communication, writing, and organizational skills
Compensation
- Annual total cash target pay: $67,905 - $101,857
- Hourly rate: $32.65 - $48.97
- Per visit point rate: $39.18 - $58.76
Compensation is based on full-time employment (40 weekly hours) at full productivity and varies based on local labor markets, education, work experience, and may increase over time based on productivity and performance. This role uses two types of compensation: per visit point pay when conducting visits and hourly pay for non-visit activities such as orientation. Per visit pay is calculated by multiplying your per visit point rate by the productivity points accrued for various types of visits. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed.
