Registered Nurse (Home Health)

Hale Makua Health Services Home Health is seeking a Registered Nurse to provide skilled nursing care to homebound patients in their homes. You will work directly with patients, families, physicians, and an interdisciplinary team using a case management model to deliver personalized care for individuals with medical conditions requiring substantial and specialized nursing skills.

Responsibilities

• Provide skilled nursing care directly to patients in their home setting
• Complete comprehensive history and physical assessments upon patient admission
• Consult with and educate patients and families on health and wellness in the home environment
• Provide leadership for the interdisciplinary team and direct care toward individual patient goals
• Promote effective and collaborative communication between all disciplines involved in patient care

Required Qualifications

• Graduate of an accredited school of nursing
• Current Registered Nurse license in the State of Hawaii
• BLS Certification

Preferred Qualifications

• At least two years of experience in a hospital setting
• Medicare Home Health Agency experience
