McLaren Home Health is seeking a Registered Nurse to provide coordinated care to an assigned group of patients in the home health setting. You will be responsible for the quality of patient care, indirect supervision of nursing personnel, and care coordination from admission through discharge.
Responsibilities
- Perform basic patient care procedures and techniques competently, including medication administration
- Organize patient care effectively and efficiently
- Conduct ongoing and systematic assessment data collection, focusing on the patient's age, physiologic, psychological, and cognitive status
- Complete, review, and evaluate assessment data collected by caregivers
- Develop individualized plans of care that prescribe nursing actions to achieve patient goals
- Identify patient and family learning needs and implement appropriate education processes from admission to discharge
- Indirectly supervise all nursing personnel assigned to your patient group
Schedule
- Part-time position
- 48 scheduled hours per bi-weekly pay period
- Daily work times: 8:00 AM to 5:00 PM
- Days shift
- Weekends required
- No on-call duties
Required Qualifications
- Active RN licensure in Michigan
- Graduate of an approved school of nursing
- One year of experience in home health or related clinical specialty
- Basic Cardiac Life Support (BCLS) certification
- Advanced Cardiac Life Support (ACLS) certification (required for certain units)
- Current Michigan driver's license
- Proof of valid automobile insurance
- Clear Motor Vehicle Report (MVR)
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
