Registered Nurse - Home Health

Jet Health, Inc / Klarus Home Health & Hospice is seeking Registered Nurses to provide home health care to community-based patients in Granbury, Weatherford, TX and surrounding areas. This position offers flexibility with full-time, part-time, and PRN employment options. You will deliver direct patient care in patients' homes, focusing on geriatric populations who require support to maintain safe, independent, and dignified living.

Responsibilities

• Follow designated care plans according to patient needs
• Make home visits to clients in assigned geographic territories
• Administer medications, perform wound care, and execute treatments and procedures
• Monitor client conditions and report changes to the Clinical or Client Services Manager
• Execute and document physicians' orders
• Perform client assessments
• Provide case management and coordinate care
• Document observations, interventions, and evaluations using a state-of-the-art touch pad tablet

Required Qualifications

• Current State or Compact Registered Nurse (RN) License
• Valid state driver's license
• Automobile liability insurance
• Demonstrate knowledge and skillset in current nursing practice

Preferred Qualifications

• Minimum one year of clinical experience
• OASIS knowledge and/or end-of-life care experience
• Knowledge of HCHB EMR

Travel Requirements

• Must travel to patients' homes in designated geographic territories
• Personal vehicle required with valid driver's license and automobile liability insurance
