Registered Nurse Home Health

Intermountain Home Health is seeking a Registered Nurse to provide comprehensive care to adult and pediatric patients and their families in home settings. As an RN with Home Health, you will assume responsibility for assessing, planning, implementing, and evaluating patient care while working autonomously in patients' homes. You will utilize the nursing process, evidence-based practice, and therapeutic interventions to deliver quality clinical care in compliance with home health policies, procedures, and regulatory standards. Responsibilities

• Assess, plan, implement, and evaluate care for home health patients and their families
• Provide direct patient care including wound care, medication and injection administration, and IV monitoring
• Deliver patient and caregiver education
• Coordinate care with other members of the healthcare team
• Document patient care and maintain accurate medical records using technology
• Work autonomously while maintaining communication with the collaborative healthcare team
• Manage patient care in compliance with home health policies, procedures, and legal and regulatory standards

Clinical Setting

You will work in patients' homes throughout the South Jordan area, providing care in a home-based setting rather than a traditional clinical facility.

Required Qualifications

• Current RN license in Utah
• Basic Life Support (BLS) certification for healthcare providers
• Current driver's license with an acceptable driving record
• Reliable, insured personal vehicle for transportation to patient homes
• Ability to communicate effectively both verbally and in writing
• Bachelor's degree in Nursing (BSN) from an accredited institution (required as of April 27, 2025; employees hired prior to this date will follow requirements in place at time of hire)
• RNs with less than 12 months of working experience as an RN prior to joining Intermountain must obtain their BSN within 4 years of their start date

Preferred Qualifications

• Two years of experience working as an RN
• Previous experience in an acute care, home health, or hospice setting

Schedule

• 4 scheduled weekly hours
• Every other Saturday 8 AM to 4 PM
• Rotating holidays

Compensation and Benefits

• Hourly rate: $35.22 to $53.29 (based on experience)

Physical Requirements

• Ability to see and read information, labels, and assess patient needs; operate monitors; identify equipment and supplies
• Frequent verbal communication with patients, families, and healthcare providers; ability to hear and understand spoken information, alarms, and needs quickly and accurately, particularly during emergencies
• Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision; frequent computer use and typing for documentation
• Ability to lift and utilize full range of movement to transfer patients; bend to retrieve, lift, and carry supplies and equipment of varying weights, including heavy items
• Ability to walk and assist with transporting and ambulating patients; push and pull gurneys and portable equipment, including heavy items
• Ability to navigate crowded and busy rooms with equipment and power cords
• May be required to stand in a stationary position for extended periods
• Ability to drive a vehicle, including sitting, seeing and reading signs and traffic signals
