Registered Nurse - Home Health

We are seeking a Registered Nurse to provide skilled nursing care to clients in their homes. As a Home Health Nurse, you will deliver direct patient care, coordinate services, and supervise Licensed Practical Nurses and Home Health Aides. You will work with clients of all ages, developing individualized care plans and teaching patients and caregivers appropriate care skills. This role requires strong clinical judgment, excellent communication, and the ability to work independently in clients' homes across the service area.

Responsibilities

• Coordinate patient care assignments and referrals, ensuring patient needs are continually assessed and met with individualized, updated care plans
• Provide direct skilled nursing care and teach patient care skills to clients and caregivers
• Develop and revise plans of care based on initial and ongoing assessments, client history, nursing diagnoses, physician orders, and client goals
• Initiate preventative and rehabilitative nursing procedures individualized to the client and age-specific needs
• Advocate for clients and families while following visit utilization guidelines and protocols
• Formulate and implement appropriate teaching plans
• Communicate significant clinical findings and changes in status to physicians and involved personnel in a timely manner
• Document all skilled and medically necessary care thoroughly, timely, and accurately in accordance with home care policy and regulatory requirements
• Conduct supervisory visits for Home Health Aides and LPNs in accordance with agency policy
• Prepare and update Home Health Aide written instructions and document findings
• Ensure delegated duties and activities remain within the scope of the Nurse Practice Act
• Participate in staff meetings, case conferences, business development meetings, and customer service programs
• Work on-call schedule as required to meet agency goals and community obligations
• Actively participate in the Quality Management program and complete required in-service training
• Deliver high-level customer service to internal and external customers

Physical Requirements

• Ability to perform heavy lifting, exerting 50 to 100 pounds of force occasionally and 25 to 50 pounds of force constantly
• Frequent to constant walking, standing, sitting, reaching, stooping, squatting, and kneeling throughout the workday
• Ability to speak, read, and write in English
• Ability to see, hear, use fine motor skills, eye-hand coordination, and tactile abilities
• Ability to follow multiple verbal and written instructions
• Potential for contact with blood, body fluids, or tissues; appropriate protective measures required

Required Qualifications

• Graduate of an approved school of nursing
• Current RN license to practice in the State of Kansas
• Current CPR certification
• Valid State Driver's License
• Proof of automobile insurance meeting at least minimum state mandated requirements
• Access to reliable transportation to serve all clients and areas serviced by the agency
• Ability to assess client status quickly and accurately and take appropriate action
• Effective verbal and written communication skills
• Working knowledge of the Windows environment, email, and word processing
• Ability to function successfully in a team environment
• Ability to problem solve and suggest solutions
• Ability to function with minimal supervision

Preferred Qualifications

• At least one year of medical-surgical nursing experience in acute care or home health setting within the past four years
• At least one year of home health experience
