Registered Nurse Home Health Hiring Now

Granite Peak Home Health is seeking a Registered Nurse to provide skilled nursing services to patients in their homes. You will conduct initial evaluations, develop and revise plans of care, perform specialized nursing procedures, and coordinate services with an interdisciplinary team. This role requires independent judgment, strong clinical assessment skills, and the ability to work autonomously in a home-based setting.

Responsibilities

• Provide nursing services according to the established plan of care
• Conduct initial patient evaluations and regularly reassess nursing needs
• Initiate plans of care and make necessary revisions based on patient condition
• Perform nursing procedures requiring specialized skills
• Implement preventive and rehabilitative nursing procedures
• Prepare clinical and progress notes for each patient visit and care conference summaries promptly
• Coordinate services with other healthcare providers
• Communicate changes in patient condition and needs to appropriate personnel
• Counsel patients and families on meeting nursing and related needs
• Participate in and present in-service programs
• Process physician orders and notify physicians of patient needs and condition changes
• Complete certification, recertification, and discharge documentation
• Determine the type and amount of nursing care needed for each patient
• Refer patients to physical therapists, speech language pathologists, occupational therapists, and medical social workers as appropriate
• Supervise and teach other nursing personnel
• Conduct patient care conferences for assigned patients
• Participate in peer review, quality assurance, and performance improvement activities
• Provide total patient care as needed
• Take on-call duty nights, weekends, and holidays as assigned
• Complete OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change documentation within agency-defined timeframes

Required Qualifications

• Graduate of an approved school of professional nursing
• Currently licensed as a Registered Nurse in Montana
• Two years of nursing experience
• BLS/CPR certification
• Ability to exercise initiative and independent judgment
• Familiarity with healthcare regulations, HIPAA compliance, and patient privacy requirements
• Knowledge of local healthcare market and community outreach strategies
• Proficiency with Microsoft Word, Excel, Outlook, and Wellsky EHR

Preferred Qualifications

• One year of home health nursing experience
• Clean driver's license with clean driving record and current vehicle insurance

Schedule

• Monday to Friday
• Weekends as needed
• On-call duty nights, weekends, and holidays as assigned
