Brooks Home Health is seeking a full-time Registered Nurse to provide nursing care planning and management for patients in the Daytona and surrounding areas. This non-supervisory position follows the Florida Nurse Practice Act and involves independent nursing practice in the home health setting.
Clinical Setting
You will work in home health, delivering patient-centered care to individuals with disabilities and chronic conditions. You will function independently and effectively, serving as a resource to the interdisciplinary healthcare team while maintaining professional communication and advocacy for your patients.
Responsibilities
- Conduct bio-psycho-social-spiritual assessments and identify actual and potential nursing and clinical problems
- Initiate and establish comprehensive individualized nursing plans of care within 24 hours, reflecting admission through discharge planning
- Develop patient-centered goals in collaboration with clients, families, and the rehabilitation team that promote wellness, independence, and socialization
- Recognize and document subtle changes in patient condition, including complex situations, and take appropriate nursing action
- Complete medication reconciliation at each skilled nursing visit and update the clinical team and physician of any changes, noncompliance, or side effects
- Administer medications and treatments as ordered, evaluate patient response, anticipate adverse reactions, and document appropriately
- Independently complete nursing skills and interventions according to specialty competency-based skills checklists
- Evaluate outcomes of care and revise the plan of care to achieve desired outcomes
- Complete all OASIS documentation according to guidelines in a timely and effective manner
- Develop and document teaching and discharge plans addressing complex patient and family needs
- Apply nursing research and evidence-based practice guidelines to clinical practice
- Demonstrate therapeutic and professional communication skills with patients, families, and healthcare team members
- Function as a patient advocate and collaborate with the interdisciplinary team in care planning and delivery
- Participate in interdisciplinary team conferences and case conferences
- Identify and intervene for peer, patient, and family education needs
- Support orientation for new staff members and ancillary personnel as a preceptor
- Maintain professional development and skills appropriate to practice and experience
- Participate in on-call and weekend rotations based on office needs
- Participate in all staff and team-based meetings and committees
- Complete all required mandatory in-services
- Adhere to policies and procedures regarding universal precautions, patient rights, attendance, conduct, grooming, and dress code
- Maintain a clean and safe environment and identify and report hazards
- Participate in performance improvement activities
- Promote a positive image of Brooks Home Health to outside agencies and the public
Required Qualifications
- Active Registered Nurse license from the State of Florida
- Associate, Bachelor's, or Master's degree from an accredited school of nursing
- Basic Life Support (BLS) certification
- Responsibility for maintaining BLS certification and required continuing education and unit-specific credentials
