Elara Caring is seeking a Registered Nurse to provide home health care to patients in their homes. You will deliver direct patient care, conduct comprehensive assessments, develop and implement individualized plans of care, and coordinate with the interdisciplinary team to ensure high-quality outcomes for more than 60,000 patients served daily.

Responsibilities

  • Assess assigned caseload of home health patients and families to identify physical, psychosocial, and environmental needs through documentation review, clinical records, interdisciplinary team reports, and on-site evaluations
  • Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment
  • Assess patient needs and obtain data on physical, psychological, social, and spiritual factors that may influence patient, family, and caregiver health status
  • Make initial evaluations and re-evaluate patient nursing needs during each visit, with follow-up visits according to patient, family, and facility needs
  • Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved in patient care
  • Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and team members
  • Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
  • Ensure all activities align with the vision of Elara Caring's board of directors, executive team, and home health leadership

Required Qualifications

  • Current, unrestricted state RN license
  • One or more years of experience in a clinical care setting as a nurse
  • Valid driver's license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Willingness to travel 50% within branch or office coverage area

  • Associate's Degree or Bachelor's Degree in Nursing
  • Previous home health or hospice experience
  • Previous experience with HomeCare HomeBase

  • Work autonomy and flexible schedules
  • Must be able and willing to travel 50%
  • Must be able and willing to travel within branch or office coverage area

Reporting Structure

You will report to the Clinical Manager or Clinical Supervisor.
