Registered Nurse - Home Health Case Manager

Saint Luke's Home Health is seeking a Full-Time Day Registered Nurse to deliver care to patients in their homes across multiple counties in Kansas and Missouri. As a Home Health RN, you will function independently while working under physicians' direct supervision, providing skilled nursing care in the home setting.

Responsibilities

• Utilize the nursing process throughout provision of patient and family care
• Coordinate patient care and consult with other disciplines of the healthcare team
• Facilitate patient care by anticipating needs, individualizing care plans, and adjusting or modifying the nursing plan of care as appropriate
• Collaborate with medical staff and other disciplines to provide and enhance patient care
• Adhere to all nursing policies and standards while proactively maintaining and improving quality outcomes

Schedule

• Monday through Friday, 8:00 AM to 5:00 PM with flexibility
• Occasional weekends required

Required Qualifications

• Active Registered Nurse license in Missouri and Kansas or Compact license
• Basic Life Support (BLS) certification from American Heart Association or Red Cross
• Valid driver's license
• One year of nursing experience
• Associate Degree in Nursing
