Job Description

St. Mary's Home Care and Hospice is seeking a Registered Nurse to provide one-to-one, compassionate care to patients in their homes. This full-time day shift position offers flexible scheduling opportunities and a $10,000 sign-on bonus with benefits starting on day one.

As a Home Care RN, you will use advanced remote monitoring technology and clinical knowledge to deliver exceptional care to patients requiring one-to-one attention and monitoring in their home environment. You will be responsible for patient assessment, care planning, intervention, and overall supervision of patient care and outcomes. Our integrated virtual care program, Home Care Connect, provides 24/7 access to our Virtual Care Center RNs and helps patients avoid preventable ER visits and hospitalizations.

Responsibilities

  • Provide one-to-one care with patients in their homes
  • Conduct client assessments and develop care plans
  • Implement nursing interventions and monitor patient outcomes
  • Supervise overall patient care in compliance with physician orders
  • Follow the Georgia Nurse Practice Act and all applicable licensure and certification requirements
  • Use advanced remote monitoring technology to enhance clinical care

The organization provides skilled nursing, physical and occupational therapy, speech therapy, medical social work, hospice, and bereavement services in patients' homes.

Required Qualifications

  • Graduate of an approved nursing education program
  • Active Registered Nurse license in the State of Georgia
  • One year of professional acute care nursing experience

  • Home care experience
