Clinical Setting
You will work as part of Henry Ford Health's Community Care Services, delivering essential health services directly to patients in their homes across the East Locations service area. This role focuses on meeting patients where they are and providing care coordination and support across diverse populations.
Responsibilities
- Provide direct patient care in the home, applying nursing principles to assess and meet patient needs
- Assess patient needs and develop individualized care plans
- Educate patients and caregivers on health management and self-care
- Coordinate with allied health professionals to ensure integrated and comprehensive care delivery
- Document patient care electronically using computer systems
- Communicate effectively with patients, families, and healthcare personnel
- Transport and move clients as needed to ensure safe and effective care
Required Qualifications
- Graduation from an accredited Registered Nurse program
- Current licensure as a Registered Nurse in the State of Michigan
- Ability to effectively communicate with patients, families, and healthcare personnel
- Knowledge of computer use for electronic documentation
- Current driver's license with a satisfactory driving record
- Reliable transportation
- Ability to lift, carry, transport, and move clients as needed
Preferred Qualifications
- Minimum of nine months to one year of clinical nursing experience
