This position offers a career opportunity in home health nursing, serving patients in Bergen and Essex Counties. As a Registered Nurse in Home Care, you will plan, organize, and direct home care services with an emphasis on community health education. You will build from community resources to plan and direct services meeting the needs of individuals and families within their homes.
Responsibilities
- Complete initial assessments of patients and families to determine home care needs, including comprehensive physical assessments and histories of current and previous illnesses
- Regularly re-evaluate patient nursing needs and adjust care accordingly
- Initiate plans of care and make necessary revisions as patient status and needs change
- Use health assessment data to determine nursing diagnoses
- Develop care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions, including patients and families in the planning process
- Supervise LPN and HHA documentation of care and services rendered according to agency policies and procedures
- Provide health care instructions to patients as appropriate per assessment and plan of care
- Identify discharge planning needs as part of care plan development and implement prior to patient discharge
- Report incidents timely and to appropriate persons per agency policy
- Communicate with physicians regarding patient needs and report any changes in patient condition; obtain and receive physician orders as required
- Participate in case conferences with other disciplines to coordinate plans of care
- Participate in on-call duties as needed
Required Qualifications
- State Certified RN license
- Experience in Certified Home Care and OASIS
- Valid driver's license and reliable transportation
- Willingness to travel
Education
- Associate's Degree in Nursing (Bachelor's preferred)
Compensation
- Salary: 90,000 to 110,000 annually
- Sign-on bonus: 3,500
