Registered Nurse (Home Health)

Acclaim Home Health seeks a Registered Nurse to provide skilled nursing care in patients' homes in the Framingham, MA area. In this role, you will deliver personalized care to help patients manage their health, achieve greater independence, and improve their quality of life while working with flexibility and autonomy over your patient caseload.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care
• Provide skilled nursing services based on each patient's needs and physician orders
• Monitor patient conditions, identify changes in health status, and communicate concerns appropriately
• Educate patients and caregivers on medications, disease management, treatment plans, and strategies for maintaining health and independence
• Collaborate with physicians, therapists, care coordinators, and other members of the interdisciplinary team
• Manage and prioritize an assigned patient caseload while maintaining high standards of care
• Complete timely, accurate, and compliant clinical documentation
• Participate in care coordination and help ensure positive patient outcomes

Required Qualifications

• Current Registered Nurse (RN) license in the applicable state
• Valid driver's license and reliable transportation
• Strong clinical assessment, critical-thinking, and decision-making skills
• Excellent communication and interpersonal skills
• Ability to work independently while collaborating effectively with a care team
• Strong organization and time-management skills
