Registered Nurse Home Health

Acclaim Home Health seeks a Registered Nurse to provide skilled nursing care in patients' homes in the Framingham, MA area. In this role, you will conduct comprehensive nursing assessments, develop individualized care plans, and deliver high-quality care based on patient needs and physician orders. You will manage your own patient caseload with autonomy while collaborating with an interdisciplinary team to ensure positive patient outcomes.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care
• Provide skilled nursing services based on each patient's needs and physician orders
• Monitor patient conditions, identify changes in health status, and communicate concerns appropriately
• Educate patients and caregivers on medications, disease management, treatment plans, and strategies for maintaining health and independence
• Collaborate with physicians, therapists, care coordinators, and other members of the interdisciplinary team
• Manage and prioritize an assigned patient caseload while maintaining high standards of care
• Complete timely, accurate, and compliant clinical documentation
• Participate in care coordination and help ensure positive patient outcomes

Required Qualifications

• Current Registered Nurse (RN) license in the applicable state
• Valid driver's license and reliable transportation
• Strong clinical assessment, critical-thinking, and decision-making skills
• Excellent communication and interpersonal skills
• Ability to work independently while collaborating effectively with a care team
• Strong organization and time-management skills
