Registered Nurse - Home Health

Puget Sound Home Health & Hospice is seeking multiple Registered Nurses to join our Home Health team serving Pierce County, with current focus on Buckley, Puyallup, and Graham. In this role, you will provide skilled nursing care to patients in their homes, building meaningful one-on-one relationships while maintaining the support of a collaborative clinical team.

Responsibilities

• Assess patients in their homes and identify individual care needs
• Develop and manage personalized plans of care in collaboration with providers
• Provide skilled nursing care, education, and support to patients and families
• Coordinate care with therapists, physicians, and other members of the interdisciplinary team
• Monitor patient progress and adjust care as needs change
• Complete accurate, timely clinical documentation
• Participate in quality initiatives to improve patient care

Required Qualifications

• Current Registered Nurse (RN) license
• Current AHA BLS certification
• Valid driver's license, insured vehicle, and reliable transportation
• Ability to work independently while collaborating with a clinical team

Preferred Qualifications

• Home Health and/or Hospice experience

• Hourly rate: $45–$60/hour, based on experience
