The opportunity

Job Description

AdventHealth is seeking a Registered Nurse for Home Health to provide skilled nursing care to patients in their homes across the greater Orlando area. You will evaluate home environments for safety and infection control, implement individualized care plans, coordinate with physicians and healthcare teams, and use health coaching techniques to engage patients and families in their care management.

Clinical Setting

You will work with patients receiving home-based care across six service teams covering areas including Altamonte Springs, Winter Park, Orlando, Sanford, Lake Mary, and surrounding communities. You will manage a caseload of home patients and optimize daily schedules to support productivity and best practice visit utilization.

Responsibilities

  • Evaluate home environments for safety, infection control, and community resource needs
  • Review patient history, physical diagnostics, and laboratory data; report abnormal results to the physician
  • Implement care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities
  • Provide skilled nursing care, preventative and rehabilitative procedures, and prescribed treatments in various home situations
  • Use motivational interviewing and health coaching techniques to engage stakeholders in care management
  • Inform the physician, clinical manager, and healthcare team of changes in patient condition and needs
  • Maintain updated clinical records, meeting documentation deadlines for certification, re-certification, and care plan updates
  • Provide comprehensive assessment, planning, implementation, and evaluation for a caseload of home patients
  • Assess physical, functional, psychosocial, cultural, and cognitive status, and discharge planning needs
  • Formulate patient-specific care plans with the patient, family, and physician, establishing individualized, realistic, measurable goals
  • Function with a high degree of independence and delegate tasks to appropriate personnel as indicated by skill level and professional standing

Schedule and Shift

  • Full-time position
  • Monday through Friday, 8:00 AM to 5:00 PM
  • Weekend on-call rotation required
  • Flexible scheduling with full-time, part-time, and PRN options available

Required Qualifications

  • Associate's degree in Nursing
  • Active Registered Nurse (RN) license
  • Valid driver's license
  • Basic Life Support (BLS) CPR certification
  • One or more years of relevant clinical nursing experience
  • Strong computer and technology skills
  • Ability to function with a high degree of independence
  • Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing

Preferred Qualifications

  • Bachelor's degree in Nursing
  • Recent, relevant experience in a Medicare-certified home health agency as a case manager
  • Working knowledge of community resources and ability to refer patients and families appropriately
  • Knowledge of home care regulations and third-party reimbursement as it impacts care delivery
  • Current IV therapy skills
  • Certified Oasis Specialist-Clinical (COSC) certification

Compensation and Benefits

  • Pay range: $31.85 to $52.76 per hour
  • Mileage reimbursement: $0.76 per mile

Additional Requirements

  • Florida Level 2 background screening, including fingerprinting, as required by state law
  • Valid driver's license required for travel to patient homes
