UMass Memorial Health is seeking a Registered Nurse for Home Health to provide direct nursing care to patients in their homes across the lifespan. This position offers the opportunity to work autonomously while delivering comprehensive, holistic care that addresses patients' medical, psychosocial, and family needs. You will coordinate care delivery, manage patient assessments, develop individualized care plans, and collaborate with patients, families, and interdisciplinary teams to promote health and safety in the home environment.
Clinical Setting
This is a home health nursing role serving patients in their homes on a 7-day per week basis. The position embraces primary, secondary, and tertiary prevention, assisting patients and families with disease and disability management, community resource coordination, and health insurance navigation.
Responsibilities
- Provide direct nursing care and case coordination to patients and families, including day, evening, night, weekend, and holiday duty
- Collect comprehensive assessment data pertinent to patients' health and situations
- Analyze assessment data to determine diagnoses and issues
- Develop individualized care plans that prescribe strategies and alternatives to attain expected outcomes
- Implement individualized patient plans of care
- Coordinate care delivery across providers and settings
- Employ strategies to promote health, comfort, and safe environments
- Evaluate progress toward attainment of outcomes
- Meet established patient care productivity standards
- Systematically enhance the quality and effectiveness of nursing practice
- Maintain current knowledge and competency reflecting current nursing practice
- Evaluate own nursing practice in relation to professional standards, guidelines, statutes, rules, and regulations
- Integrate ethical principles and best practices into all areas of practice
- Collaborate with patients, families, peers, and colleagues in the conduct of nursing practice
- Provide leadership in the professional practice setting
- Maintain organized, clean, and safe work environments
- Participate in quality improvement initiatives, staff meetings, and in-service programs
- Adhere to departmental policies, procedures, and the organization's Code of Conduct and CARES values
- Practice cost containment and fiscal responsibility
Required Qualifications
- Diploma or Associate's Degree in Nursing
- Valid and unrestricted licensure as a Registered Nurse in the Commonwealth of Massachusetts
- Valid and unrestricted driver's license recognized by the Commonwealth of Massachusetts
- Access to a reliable, insured vehicle
- Ability to perform all duties according to American Nurses Association Scope & Standards of Practice for Home Health Nursing and agency policies and procedures
- Ability to use a laptop computer and proficiently utilize or learn to utilize MS Office applications, email, Point-of-Care software applications, and wireless applications
- Ability to perform comprehensive physical and psychosocial assessments
- Ability to work autonomously
- Ability to communicate professionally and effectively with a diverse patient population
- Ability to read, write, and speak clearly in English
Preferred Qualifications
- Bachelor's, Master's, or Doctorate in Nursing
- Specialty nursing certification as a home health nurse, hospice and palliative care nurse, or case manager
- Completion of the Certified OASIS Specialist certificate
- 2-5 years of recent (within the past 18 months) medical-surgical experience in an acute or subacute setting, or equivalent experience working in a certified home health agency or licensed hospice
- Previous case management or program management experience
Schedule
- Monday through Friday, plus Weekends (Saturday and Sunday)
- 8:00 AM to 4:30 PM
- 40 hours per week

Compensation
- Hiring range: $37.38 - $62.22 per hour (based on a full-time schedule of 40 hours per week)
- Final offer will vary based on scheduled hours, experience, skills, qualifications, and internal equity considerations
- Compensation for part-time or reduced schedules will be prorated based on scheduled weekly hours
- Signing bonus may be available; eligibility will be confirmed during the interview process
