Registered Nurse - Home Care/Long Term Care - Hospice of Cincinnati

Join Hospice of Cincinnati as a Registered Nurse supporting patients and families during end-of-life care. In this field-based home care role, you will provide direct and indirect holistic care, serve as a resource for nurses, physicians, caregivers, and loved ones, and help maintain comfort, dignity, and continuity of care.

Clinical Setting

This position operates within a hospice care team model, requiring independent work in patients' homes and community settings. You will collaborate with physicians and other healthcare team members in a collegial manner to meet patient and family needs.

Responsibilities

• Perform initial and ongoing assessments of patients and families, completing assessment tools and documenting findings per TriHealth guidelines
• Communicate assessment findings to other healthcare providers as appropriate, including health counseling and teaching needs
• Plan individualized care for patients and families based on assessment, standards of care, and optimal outcomes
• Initiate and individualize appropriate patient care guidelines, clinical pathways, and plans of care
• Develop short and long-term goals with patients, families, and the healthcare team, including discharge planning
• Provide a safe, therapeutic environment that maintains patient autonomy, dignity, and rights while being sensitive to patient diversity
• Recognize emergency situations and take appropriate action
• Complete patient assignments and documentation within scheduled timeframes
• Base interventions on clinical data and desired outcomes, documenting accordingly
• Train and educate other staff, serving as an expert resource in specialty areas
• Carry out physician orders and safely administer medications and treatments, monitoring their effects
• Evaluate plans of care based on optimal patient outcomes and document patient and family responses to care and teaching
• Collaborate with patients, families, and healthcare team members to revise plans of care as needed
• Supervise care delegated to other healthcare team members
• Perform technical skills according to policy, procedure, and accepted standards
• Use all equipment in a safe and appropriate manner
• Identify areas for professional and self-improvement through self-assessment and peer review
• Share knowledge and skills with colleagues and provide constructive feedback
• Maintain awareness of continuous improvement changes and support them through active participation

Required Qualifications

• Associate's Degree or Diploma in Nursing
• Active Registered Nurse license
• Basic Life Support for Healthcare Providers (BLS) certification
• Knowledge, judgment, and skills derived from principles of biological, physical, behavioral, social, and nursing sciences to meet complex healthcare needs at various stages of the life cycle
• Up to 1 year of clinical nursing experience

Preferred Qualifications

• Membership in related professional organizations

Education

• New hires are required to obtain a Bachelor of Science in Nursing (BSN) within 5 years of hire

Schedule

• Full-time position (32 hours weekly)
• Split shift: 11:00 a.m. to 11:30 p.m.
• Every third weekend required
• Rotating holidays

Travel Requirements

• This position requires travel to patients' homes
• Mileage reimbursement provided at $0.72 per mile

Working Conditions

This role involves consistent engagement in the following activities: concentrating, continuous learning, hearing conversation and other sounds, interpersonal communication, lifting (under 10 lbs., under 50 lbs., and 50+ lbs.), pulling, pushing, reaching, reading, standing, stooping, talking, thinking and reasoning, use of hands, color vision, visual acuity (far and near), and walking. Occasional activities include sitting, kneeling, and stooping. Climbing is rare.
