Elite Home Health & Hospice is seeking a Registered Nurse to provide compassionate home health and hospice care to patients in the Lewiston, ID area. In this full-time role, you will work independently in patients' homes, delivering personalized nursing care while coordinating with an interdisciplinary team to support patients and families through critical health transitions.
Clinical Setting
You will provide direct nursing care in home and community settings, serving patients with varying acuity levels and care needs. This position emphasizes community health education and leverages community resources to support individual patients and families.
Responsibilities
Patient Care
- Complete initial, comprehensive, and ongoing assessments of patients and families to determine home health or hospice needs
- Provide complete physical assessments and detailed histories of current and previous illnesses
- Develop and implement individualized care plans based on nursing diagnoses, incorporating palliative nursing actions and patient/family goals
- Regularly re-evaluate patient and family/caregiver needs and revise the care plan as necessary
- Administer medications and treatments as prescribed by the physician
- Initiate appropriate preventive and rehabilitative nursing procedures
- Counsel patients and families on meeting nursing and related needs
- Provide health care instructions appropriate to patient assessment and plan of care
- Assist patients with activities of daily living while facilitating self-sufficiency and comfort care
- Act as case manager and coordinate patient care for your assigned caseload
Communication and Coordination
- Complete, maintain, and submit accurate and relevant clinical notes regarding patient condition and care provided
- Record pain and symptom management changes and outcomes
- Communicate with physicians regarding patient needs and report changes in patient condition; obtain and receive physician orders as required
- Communicate with community health partners to coordinate the care plan
- Teach patients and families/caregivers self-care techniques and provide medication, diet, and other instructions as ordered
- Work collaboratively with the interdisciplinary team
- Attend interdisciplinary group meetings
- Assist patients, families, caregivers, and team members in providing continuity of care
- Work cooperatively with families/caregivers and hospice team members to meet emotional needs
Additional Duties
- Participate in on-call duties as defined by agency policy
- Ensure arrangements for equipment and necessary items and services are available
- Supervise ancillary personnel and delegate responsibilities when required
- Maintain and upgrade professional knowledge and practice skills through continuing education and in-service participation
- Fulfill requested and accepted case assignments
- Actively participate in quality assessment and performance improvement activities
Required Qualifications
- Graduate of a National League for Nursing accredited school of nursing
- Current RN licensure to practice professional nursing in Idaho
- Current CPR certification
- Minimum of two years of nursing experience
- Ability to organize and prioritize workload independently
- Excellent observation, problem-solving, verbal, and written communication skills
- Nursing skills per competency checklist
- Valid driver's license with current automobile insurance
- Access to a personally owned automobile for agency-related travel to community partners and patients
- Ability to drive and maintain reliable transportation as an essential function of the job
- Physical ability to perform prolonged or considerable walking or standing
- Ability to lift, position, and transfer patients
- Ability to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling, and crouching
- Visual acuity and hearing sufficient to perform required nursing skills
- Self-directed and able to work with minimal supervision
Preferred Qualifications
- Minimum of one year of experience in public health, home care, or hospice nursing
Compensation and Benefits
- Hourly pay: $38 - $43, depending on experience
