Registered Nurse for Home Health

AdvisaCare is seeking Registered Nurses to provide skilled nursing care to Medicare beneficiaries in their homes, typically following inpatient stays for illness or surgery. As a home health RN, you will help patients recover their independence and manage their care in a home setting.

Responsibilities

• Assess and evaluate patient health status and home safety
• Help patients understand and manage their disease process
• Perform skilled nursing treatments including wound dressing and care
• Draw blood and manage drains, catheters, PICCs, and ostomies
• Support medication management and disease management education
• Connect patients with community resources as needed
• Coordinate care with physicians, providers, caregivers, therapists, and families

Required Qualifications

• Active Michigan RN license
• Valid Michigan driver's license and proof of insurance
• Current CPR certification
• Critical thinking and task management skills
• Computer skills

Preferred Qualifications

• One year of home health nursing experience
• Emergency department or critical care experience
• OASIS experience
