VitaCare Living is seeking an experienced Registered Nurse to serve as an RN Consultant supporting assisted living and memory care communities in Stratford, Wisconsin. In this part-time contracted role, you will provide clinical oversight, regulatory compliance support, staff education, and quality improvement initiatives to ensure residents receive safe, respectful, individualized care. You'll work with community leaders and caregivers to strengthen clinical practices, maintain survey readiness, and meet Wisconsin DHS 83 requirements.
Clinical Setting
This position supports VitaCare Living's assisted living and memory care communities. VitaCare Living operates 17 locations across two states, serving over 300 residents in smaller, community-focused settings.
Responsibilities
- Complete resident assessments and update Individual Service Plans (ISPs)
- Review psychotropic medications, assess wounds and skin conditions, and follow up on changes in condition and new medical orders
- Conduct medication cart audits and Medication Administration Record (MAR) reviews
- Reconcile medications with physician orders and address discrepancies or errors
- Delegate medication administration to qualified staff and verify competency
- Provide hands-on education to reinforce safe medication practices and the Five Rights
- Audit resident records, staff training files, incident reports, and emergency preparedness documentation for compliance with DHS 83 and VitaCare policies
- Support survey preparation and develop and implement plans of correction in response to Statements of Deficiencies (SODs)
- Educate staff and coach community leaders
- Provide clear written findings and recommendations after each visit
- Identify opportunities for clinical improvement and help communities implement sustainable practices
Required Qualifications
- Active Registered Nurse (RN) license valid in Wisconsin
- Experience in assisted living, Community-Based Residential Facilities (CBRFs), memory care, or long-term care
- Working knowledge of Wisconsin DHS 83 regulations and the assisted living survey process
- Experience with medication management, RN delegation, and staff competency assessments and training
- Strong communication and coaching skills with the ability to translate findings into practical action plans
- Reliable transportation and willingness to travel to VitaCare Living communities as needed
- Ability to pass a caregiver background check
Preferred Qualifications
- Clinical auditing, quality assurance, or regulatory compliance experience
Schedule
- Part-time, approximately 12 hours per week minimum
- Combination of on-site community visits and remote work based on community needs
