We are seeking a Registered Nurse to join our Community Treatment Team, providing clinical treatment services to individuals with serious mental illness and/or substance use disorders. In this role, you will deliver direct patient care, medication education, outreach, case management, and advocacy services across diverse community settings including streets, shelters, jails, hospitals, homes, and rehabilitation facilities. Approximately 90% of your work will be conducted in the community.

Responsibilities

  • Deliver and observe medication administration; educate patients on medications and side effects
  • Provide outreach, case management, counseling, and advocacy services in any environment as clinically indicated
  • Develop individualized treatment plans based on consumer stage of change and identified needs
  • Implement stage-wise treatment interventions and exercise sound clinical judgment in routine, urgent, and emergent situations
  • Link individuals to essential services and resources
  • Collaborate with the treatment team to support consumer-first approaches grounded in trust and respect
  • Maintain extensive knowledge of Medicaid and Department of Behavioral Health and Developmental Services policies and regulatory guidelines

Required Qualifications

  • Valid Pennsylvania Registered Nurse license
  • Valid driver's license and ability to drive an organization vehicle
  • One of the following:
  • Bachelor of Science in Nursing (BSN) and one year of nursing experience, OR
  • RN license and two years of nursing experience
  • Working knowledge and understanding of medications used in treating psychiatric disorders
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Proficiency in computer documentation and electronic health record systems
  • Flexibility to adapt to changing environments and patient needs
  • Act 33/34/74 Clearances (Criminal/Child/FBI Department of Human Services)
  • Child Protective Services Law (CPSL) training
  • Pre-employment drug screen

  • Medical experience

Schedule

  • Full-time, exempt (salary) position
  • Primarily Monday through Friday, daylight hours
  • Rotating on-call responsibilities
