Registered Nurse Clinical Manager - Home Health

Seaport Scripps Home Health, a Scripps Health joint venture, is seeking an experienced Registered Nurse Clinical Manager to oversee clinical operations and support field clinicians in delivering high-quality, coordinated home health care. This full-time role works closely with physicians, patients, caregivers, and the interdisciplinary team to ensure appropriate care and strong clinical outcomes.

Clinical Setting

This position is based in home health care operations, where you will coordinate patient care across the agency's service area and collaborate with the Director of Nursing and agency leadership.

Responsibilities

• Coordinate and oversee home health patient care
• Supervise and support clinical staff
• Develop, review, and update individualized plans of care
• Review referrals and assess patient care needs
• Coordinate physician orders, medications, and clinical services
• Lead case conferences and interdisciplinary care coordination
• Review clinical documentation for quality and compliance
• Support staff orientation, education, and clinical development
• Participate in quality assurance and performance improvement (QAPI) activities
• Collaborate with the Director of Nursing and agency leadership

Required Qualifications

• Current and active Registered Nurse license in California
• Home health experience required
• Current CPR certification
• Valid driver's license, auto insurance, and reliable transportation
• Strong clinical judgment, leadership, and communication skills

Preferred Qualifications

• 2 or more years of clinical management or supervisory experience

Compensation and Benefits

• Pay Range: $120,000 - $145,000 annually
