Registered Nurse Clinical Manager - Home Health

Seaport Scripps Home Health, a Scripps Health joint venture, is seeking an experienced Registered Nurse Clinical Manager to oversee clinical operations and ensure high-quality, coordinated patient care in the home health setting. This full-time role works closely with physicians, patients, caregivers, and the interdisciplinary team to support field clinicians and improve patient outcomes.

Responsibilities

• Coordinate and oversee home health patient care
• Supervise and support clinical staff
• Develop, review, and update individualized plans of care
• Review referrals and assess patient care needs
• Coordinate physician orders, medications, and clinical services
• Lead case conferences and interdisciplinary care coordination
• Review clinical documentation for quality and compliance
• Support staff orientation, education, and clinical development
• Participate in quality assurance and performance improvement activities
• Collaborate with the Director of Nursing and agency leadership

Required Qualifications

• Current and active Registered Nurse license in California
• Home health experience required
• Strong clinical judgment, leadership, and communication skills
• Current CPR certification
• Valid driver's license, auto insurance, and reliable transportation

Preferred Qualifications

• 2 or more years of clinical management or supervisory experience

Compensation and Benefits

• Pay range: $120,000 to $145,000 annually
