MultiCare is seeking a Registered Nurse Case Manager to join our home health team serving South King County. In this role, you will facilitate interdisciplinary plans of care, guiding patients toward achieving clinical, quality, and cost-effective outcomes. You will organize and prioritize patient care, collaborate with the nursing team to improve quality and reduce costs, and advocate for ethical and holistic care for patients from diverse backgrounds.
Responsibilities
- Organize patient care by prioritizing, delegating, delivering, and evaluating on an ongoing basis
- Utilize nursing education and experience to deliver nursing care using the nursing process according to policies, procedures, and protocols
- Collaborate with the nursing team to facilitate overall improvement in quality of care, patient experience, and cost reduction
- Foster a learning environment for patients, families, nursing staff, and members of the health care team
- Advocate for ethical and holistic care promoting the autonomy, dignity, and health of patients from diverse ethnic and social backgrounds
Schedule
- Full-time (1.0 FTE)
- Monday through Friday, day shift
Required Qualifications
- Bachelor's degree in Nursing, or obtained within five years from the initial date of hire (RN with 15 or more years of service may be considered in lieu of education agreement)
- Current Registered Nurse license in Washington State or Multistate License endorsement
- Current BLS Certification
- Three years of recent clinical experience in specialty area

Compensation and Benefits
- Competitive salary range: $109,262.40 - $194,251.20 USD
- Pay influenced by skill set, level of experience, and certifications or education
- If associated with a union contract, pay will reflect the appropriate step on the pay scale based on years of experience
