Registered Nurse Case Manager - Home Health

Advocate Health is seeking a Registered Nurse Case Manager for Home Health to manage specialty patient caseloads in the Milwaukee South area. In this full-time role, you will provide comprehensive nursing care coordination, conducting initial assessments, establishing plans of care, and delivering direct patient care in home settings.

You will be responsible for ongoing patient assessment and care adjustments, maintaining accurate clinical documentation including OASIS and 485 forms, and communicating regularly with physicians and the interdisciplinary healthcare team. You will facilitate discharge planning, participate in quality improvement initiatives, and ensure continuity of care throughout the patient journey.

Clinical Setting

This position is based in home health, requiring you to travel to patients' residences throughout the Milwaukee South service area. You must possess a valid driver's license and maintain a registered, functional vehicle with adequate insurance coverage. Mileage reimbursement is provided.

Responsibilities

• Assume responsibility for specialty patient caseloads
• Conduct thorough initial assessments to obtain physical, social, and environmental data
• Establish plans of care in accordance with physician orders and patient needs
• Provide direct patient care and adjust nursing interventions as needed
• Identify problems, modify care plans, and evaluate outcomes within appropriate timeframes
• Provide ongoing assessment of patient response to treatments and teaching
• Update and revise plans of care as indicated
• Complete documentation including visit assessments, OASIS, 485 forms, and verbal orders accurately and promptly
• Communicate with physicians to confirm and update patients' plans of care
• Provide status updates and ensure ongoing communication with the healthcare team
• Facilitate discharge planning and coordinate resources to ensure continuity of care
• Participate in chart audits and continuous quality improvement activities
• Identify and participate in process improvement opportunities
• Maintain accurate clinical and patient records according to agency, state, and federal guidelines
• Demonstrate knowledge and skills appropriate to the age of patients served

Schedule

Full-time position, 40 hours per week. Standard schedule is Monday through Friday, 7:30 AM to 4:30 PM, with occasional holiday, weekend, and evening on-call hours.

Required Qualifications

• Active Registered Nurse license issued by the state in which you practice
• Valid driver's license issued by the Division of Motor Vehicles or the state of Illinois
• Associate's Degree in Nursing
• Typically 3 years of experience in clinical nursing
• Excellent communication and strong interpersonal skills
• Ability to work with a variety of clients with patience and respect
• Good organizational, analytical, and problem-solving skills
• Demonstrated ability to work well with physicians and other professionals in a direct and positive manner
• Proficiency in clinical skills with the ability to work under direction and make sound judgments
• Ability to operate necessary equipment including FAX, computer, copier, and telephone systems
• Ability to assess patient data and interpret information to identify patient-specific requirements
• Tolerance for differences and appreciation of multiculturalism and diversity
• Proof of valid driver's license and registered, functional vehicle with adequate insurance coverage

Licenses and Certifications

• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) must be obtained within 6 months unless your department leader determines it is not required

Physical Requirements and Working Conditions

This position requires the ability to:

• Perform manual dexterity required to use and operate a variety of equipment
• Move about in confined spaces, including bending, twisting, kneeling, squatting, and occasionally reaching overhead with one or both arms
• Work safely with people of various sizes and weights with diverse and complex medical conditions and physical limitations
• Sit, stand, walk, and climb stairs for limited to prolonged periods to complete paperwork, gain entry to patients' residences, travel between visits, and attend work-related activities
• Maintain physical stamina to attend to patient care requirements over three or more hours without complete rest
• Read gauges and measuring devices with accurate vision and interpret written instructions
• Lift up to 50 pounds from floor to waist
• Lift up to 20 pounds from waist to overhead
• Lift and carry up to 40 pounds at waist height for reasonable distances
• Push or pull with 30 pounds of force
• Perform a sliding transfer of 150 pounds with a second person present

You will be subject to extreme temperature changes due to weather conditions and potential exposure to chemical and hazardous waste and communicable disease.

Compensation and Benefits

Pay Range: $38.20 to $57.30 per hour, based on qualifications, skills, relevant experience, and training.
