CVS Health is seeking a Registered Nurse Case Manager to provide field-based care coordination in Northern Virginia. In this role, you will support members across Fairfax, Sterling, Reston, Ashburn, and surrounding areas through a combination of in-person visits and telephonic outreach, delivering coordinated, patient-centered care that addresses medical, behavioral, and social needs.
Responsibilities
- Conduct comprehensive assessments through both telephonic and face-to-face interactions to evaluate members' medical, behavioral, and social needs
- Develop and implement personalized care plans that promote short- and long-term wellness
- Coordinate care across providers, programs, and services to ensure seamless member experience
- Use clinical expertise and data insights to identify risks, gaps in care, and intervention opportunities
- Support members with complex or multiple conditions using a holistic, whole-person approach
- Facilitate referrals to appropriate clinical and community resources
- Participate in case conferences and collaborate with multidisciplinary teams to optimize care strategies
- Review claims history and benefit eligibility to inform care planning
- Engage members using strong interviewing skills to build trust and encourage active participation in their care
- Ensure all case management activities comply with regulatory and organizational guidelines
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license in Virginia
- Residence in Fairfax, Sterling, Reston, Ashburn, and surrounding Northern Virginia areas
- Reliable transportation and willingness to travel up to 75% of the time; mileage is reimbursed per company policy
- 3 or more years of clinical nursing experience in medical-surgical, acute care, or behavioral health
- Strong communication, organization, and critical-thinking skills
- Comfort working with technology, including multiple systems and documentation platforms

- Experience in case management, particularly within an integrated care model
- Bilingual capability (English/Spanish)

- Ability to thrive in a fast-paced, dynamic environment and manage multiple priorities effectively
Education
- Associate Degree in Nursing (required)
- Bachelor of Science in Nursing (BSN) (preferred)
Schedule
- Monday through Friday, 8:00 AM to 5:00 PM EST
- Full-time position, 40 anticipated weekly hours
- Up to 75% travel required
Compensation and Benefits
- Pay range: $60,522.00 to $129,615.00 annually
- Eligible for CVS Health bonus, commission, or short-term incentive program in addition to base pay
