Registered Nurse Case Management

This is a full-time, on-site Case Management Nurse position located in the Greater Chicago Area. The role coordinates patient care across the continuum of services, assessing clinical needs, developing care plans, and facilitating safe and timely discharge. You will work collaboratively with physicians and interdisciplinary teams to evaluate appropriate use of healthcare services, ensure adherence to clinical guidelines and payer requirements, and connect patients with community and post-acute resources.

Responsibilities

• Assess clinical needs and develop individualized care plans
• Coordinate patient care transitions and facilitate safe discharge planning
• Collaborate with physicians and interdisciplinary teams on care coordination
• Evaluate appropriate use of healthcare services and medical necessity
• Review length of stay and ensure compliance with payer requirements
• Communicate with patients, families, and caregivers to provide education and support decision-making
• Connect patients with community resources and post-acute care options
• Document case management activities and maintain accurate records
• Monitor patient outcomes and contribute to quality improvement initiatives

Required Qualifications

• Active Registered Nurse (RN) license in the State of Illinois or eligibility to obtain
• Graduation from an accredited nursing program
• Strong case management and discharge planning skills
• Utilization management experience, including review of medical necessity, length of stay, and payer requirements
• Solid nursing and medicine knowledge to support clinical assessments
• Excellent communication, documentation, and critical-thinking skills
• Ability to work collaboratively in a fast-paced clinical environment
• Familiarity with electronic health records (EHRs) and basic computer proficiency

Preferred Qualifications

• Prior experience in hospital case management, care coordination, or discharge planning
• Experience with chronic disease management
• Certification in Case Management (CCM or ACM)
