Registered Nurse Care Manager (RN) - Health Home

University Health is seeking a Registered Nurse Care Manager to join the Health Home program at Truman Medical Center in Kansas City, Missouri. In this full-time role, you will provide comprehensive care coordination and management for Health Home enrollees, helping patients navigate the healthcare system and achieve optimal health outcomes.

You will design, implement, and evaluate individualized treatment plans in collaboration with an interdisciplinary healthcare team. Your responsibilities will include tracking patient healthcare outcomes, adjusting treatment plans as needed, and using evidence-based practices to guide care decisions. You will work directly with patients to support illness management and empower them to take an active role in their healthcare journey.

Responsibilities

• Design, implement, and evaluate treatment plans in collaboration with the healthcare team
• Provide and manage care for Health Home enrollees, tracking healthcare outcomes
• Work with patients to help them manage their illnesses and navigate the healthcare system
• Coordinate patient care with other team members to foster a holistic approach to treatment and recovery
• Make adjustments to treatment plans based on patient progress and outcomes
• Use evidence-based information to drive care decisions

Schedule

• Full-time position
• Monday through Friday, 8:00 AM to 4:30 PM
• 40 hours per week

Required Qualifications

• Bachelor of Science in Nursing (BSN) or equivalent combination of Associate Degree in Nursing (ADN) and relevant healthcare experience
• Current Missouri RN license
• Current Basic Life Support (BLS) certification
• Minimum of three years of experience working in a healthcare setting
• Strong written and verbal communication skills
• Proficiency with Microsoft Word, Excel, and PowerPoint
• Ability to read and speak English fluently

Preferred Qualifications

• Experience working in community health settings
• Ability to speak multiple languages
