This position supports patient flow operations by managing admission and discharge nursing care for an assigned group of patients across a defined work period. You will serve older adolescent, young adult, adult, geriatric, and pediatric patients, focusing on expediting safe patient throughput while maintaining comprehensive care standards.

Responsibilities

  • Develop comprehensive nursing care plans addressing all significant patient needs and problems identified during admission assessment
  • Complete nursing activities associated with patient admission and discharge
  • Provide safe and therapeutic care, identifying and accessing appropriate resources
  • Collaborate with the interdisciplinary team to ensure thorough and effective patient transitions
  • Evaluate patient response to interventions and goal attainment in collaboration with the care team
  • Complete nursing documentation for admission and discharge functions
  • Assess education needs of patients and significant others, documenting their response to teaching
  • Utilize, adapt, and revise teaching resources to meet individual patient education needs
  • Explain procedures, tests, and disease processes to patients and families as needed
  • Report and document patient condition accurately and concisely to appropriate personnel
  • Maintain patient confidentiality
  • Apply verbal and nonverbal communication skills to maintain open, effective communication among care team members, patients, and significant others
  • Encourage direct discussion between staff members when differences occur and seek assistance to arbitrate as needed
  • Contribute to identification, implementation, and improvement of the throughput process
  • Give and accept constructive feedback appropriately
  • Identify actual and potentially unsafe patient care practices, report them through designated channels, and suggest alternatives
  • Perform other duties as assigned

Required Qualifications

  • Associate's Degree in Nursing
  • RN License in state of employment or compact license
  • BLS certification

Schedule

  • Monday through Friday, 8:00 AM to 5:00 PM
  • Per diem as needed
