Job Description

Vanderbilt Health is seeking a Registered Nurse 4 to join CHOC OHO Admin. This advanced practice role applies the nursing process to plan and manage care for complex patients across the continuum of care. You will work as part of a case management triad model, collaborating to develop and meet key organizational outcomes in financial performance, patient satisfaction, and clinical quality. The role emphasizes advocacy on behalf of patients, families, and caregivers to ensure safe, equitable healthcare provision, with a focus on evidence-based practice and continuous monitoring of patient outcomes.

Responsibilities

  • Collaborate in establishing plans of care, coordinate and implement care delivery while anticipating patient needs across the continuum
  • Analyze comprehensive information pertinent to patient care and management to determine nursing diagnoses
  • Utilize the nursing process to evaluate patient progress toward goals and apply appropriate interventions to ensure effective outcomes
  • Employ strategies to promote health and maintain a safe, healthy environment
  • Facilitate appropriate follow-up care and refer patients to the appropriate next level or site of care
  • Serve in an advocacy role on behalf of patients, families, and caregivers
  • Engage in learning activities according to Board of Nursing standards to further professional growth

Required Technical Capabilities

  • Advanced Nursing Patient Assessment and Evaluation: Demonstrate expertise in patient assessment and evaluation in complex situations, conduct primary care patient interviews and physical examinations, and mentor less experienced peers
  • Advanced Nursing Patient Education: Apply patient education expertise in complex situations, deliver services creatively, and contribute to development of patient education best practices
  • Advanced Care Planning: Anticipate and plan for complex resource needs, identify discrete tasks, set priorities, lead healthcare team members to identify desired outcomes, and coach less experienced peers
  • Advanced Evidence-Based Practice: Recognize, implement, and evaluate practice changes based on published research and apply evidence-based practices to complex situations

Required Qualifications

  • Active Registered Nurse license
  • Graduate of an approved discipline-specific nursing program
  • Minimum 2 years of relevant nursing work experience
